What Service Code 076 covers

CFS is designed for adults 18 and older living in the family home. Rather than replacing family care, it strengthens it: a coordinator works alongside the household to connect the consumer to community programs, teach practical independence skills, and build a support network that lasts. Services are authorized by your Regional Center Service Coordinator through a Purchase of Service, at no direct cost to your family.

Person-centered assessment and planning

We start by sitting down together in your home to listen to your story, goals, and daily routines. Everything we do is built around your choices, culture, and timeline.

  • Face-to-face assessment conducted in the primary family home

Community resource and system navigation

Applying for public programs can feel like a full-time job. We stand by your side to help you identify, apply for, and unlock the community programs your family is entitled to receive.

  • In-Home Supportive Services (IHSS) and housing support programs
  • Social Security (SSA) benefits and financial assistance
  • Local healthcare, wellness, and community centers
  • Regional transit programs and reduced-fare applications

Independent living skills and travel training

We work directly with adults to build practical, real-world confidence, so daily life extends beyond the front door.

  • Travel training: local bus routes, transit apps, and street safety practice
  • Daily routines: budgeting, scheduling appointments, managing a weekly calendar
  • Self-advocacy: expressing needs, making personal choices, building communication skills
